Summer Camp Forms

Please complete a separate form for each registered camper.

• Summer Camp 2024 Registration Form.  Online Google form for campers entering grades 1 – 7
• Summer Camp 2024 Counselor in Training (CIT) Form with Essay for kids entering grade 8 interested in becoming a CIT.
• Ossipee Children’s Fund Application.  The Ossipee Children’s Fund (OCF) is a local nonprofit and wonderful resource that assists families with the cost of childcare and recreational, enrichment, and educational programs.  Madison Recreation is happy to accept OCF grant letters for our Summer Camp program.

Volunteer Form

• Town of Madison Volunteer Form.  If you’re interested in becoming a member of the Town of Madison Recreation Committee, or you are a volunteer coach, this is the form for you.
